Sir Patrick Vallance, the government's chief scientific adviser during the pandemic, says he was concerned about the government's "operational response" in the early days of COVID.

He told the inquiry there was "a lot done" in terms of discussing options to reduce the spread of the virus, but the operational response was not "very effective" and modellers had "great difficulty" getting clarity on numbers from the NHS.
