An inquest jury has said that police made serious mistakes in the case of Gracie Spinks, who was fatally stabbed by a colleague.

Gracie, who was 23, was killed by Michael Sellers in Derbyshire in 2021 before he took his own life.

Her inquest heard that she had reported Sellers to police before her death over stalking concerns but officers had decided that he posed a low risk.
